Hello, and welcome to Tortoise Forum.
How do you mean almost no vets in the area ? If there is just one herp vet it is worth getting an appointment. Ordinary vets will probably not be of much help.
The biggest problem will be the infection risk, so you must keep the area clean and sterilized.
If it's bad, your responsibility is to find a good vet.
Here is what they need to do if it's serious.:

And another typical thread if you don't go the vets which also offers some advice:

Ok, it doesn't look as bad as I had imagined, but it's impossible to say what the internal damage is.
It won't eat for a while as it is in shock.
The good news is that they are pretty tough and can survive some appalling injuries if properly treated.
Get some Neosporin or the like and allow the tortoise to have time alone in his hide to relieve some of the stress he is feeling.
But get to a good herp vet if at all possible.
And keep him safely in his enclosure where accidents can't happen.

Hi, and welcome to the Forum!

This is just one more example of why tortoises need their own habitat. He needs to be kept in his habitat and not allowed to wander around on the floor, and don't allow children to take him out and play with him.

Set this baby up in a small plastic tub or small aquarium and add a light. Then figure out a way to cover the whole thing to keep the heat from the light inside. He needs to be kept very warm while he is getting over the stress of being stepped on. Keep the heat about 85F inside the enclosure.

Soak him in warm water every day for about 15 minutes. Don't make it too deep. Just enough water so he can hold his head comfortably above the water to breathe.

You said "turtle" but I think this is a tortoise. Can we see a better picture of the whole animal please?
