Looks like a waterlogged piece of lettuce to me. Since it is coming out of the mouth, it is possible that the tortoise burped it up and it has been partially digested which could account for the translucent appearance.

This makes the most sense, because he kept opening his mouth wide, but not eating. Until he managed to swallow the thing back down, then he ate.
