There'a nothing wrong with your tortoise's nose, however the beak is badly overgrown. Does the tortoise get any outside time so that it can graze and eat weeds and grasses? In the wild, grazing is how the tortoises keep their beak filed down where it should be. Because your tortoise's beak is so over grown, grazing or feeding it on a rough surface isn't going to help now. You will have to go the route suggested to you by coreyc, and take it to the vet.

You might consider chopping his food in smaller pieces until it is trimmed so he can eat better.
