A lot of changes are needed ASAP
First stop all the fruit, it's very bad for his gut.
Second he needs a much bigger enclosure ASAP, a 4x8 minimum
He needs proper substrate, orchid or fir bark.
A bigger clay saucer water dish he can fit into.
A basking area with an incandescent flood bulb that reaches 95-100
All over temps that ranges 75-80
And night temps into the sixties when he's better, now do not let it go below 75.
This tortoise needs these changes NOW! He has been poorly housed and fed!

Be sure if he doesn't have a uvb light that you get the tube florescent kind not any of the cheap coil type, not the halogen and not a mercury vapor bulb.
Do not ask the store employees for recommendations as they will sell you all the bad stuff.

No. The uvb light you have doesn't get warm enough.
You need a 65 watt incandescent flood bulb or regular incandescent lamp bulb for basking. It has to make the basking area reach temps of 95-100

Some people will use the ceramic heat emitter like you have. But place it right by the light of it isn't. They bask in the wild by the sun so they look for lighted heat to bask in

You will have to build an enclosure as you won't be able to find a proper one at the pet store. You can try looking for a raised garden bed container to use, a large animal stock watering tank to use or a pop up greenhouse to use, all to be used indoors
