Take the worm and a stool sample to a local reptile vet. You will need the correct worming medication in the correct dose. Different meds for different worms.
Don't let the vet give your tortoise any "vitamin injections".
I do not recommend trying to self diagnose, or medicate with out the advice of a qualified reptile vet. Panacur works for some worms, but not others. You need the correct medicine for the species of worm you have, and you need the correct dose for your individual tortoise.I saw another forum and it said about Panacur. I have an eastern hermanns tortoise. The vets around me are close today ;(

Tortoises typically react very badly to vitamin shots. Excess vitamin A in particular causes horrendous skin problems.
Keep a close eye on your tortoise. And find another vet - one that actually knows about tortoises!
