Neck marks (Red-footed)

My tortoise, Tim (Redfoot), has some marks on his neck that I'm worried about. Can anybody shed some light about what these are? Has anybody dealt with these before? That's a bad place for an injury. It continually cracks open as the tortoise brings his head in and out and the skin folds and stretches. Your best bet is to keep it softened by keeping Neosporin on it.

Mainly how this happens is the tortoise is climbing and has his neck stretched out. He loses his hold, quickly slips back down, and doesn't bring his head in fast enough to avoid the injury from his shell. Looks like your tortoise may have done it twice.
