It would depend on the tortoise and the waste production.
I guess if you spot clean and for whatever reason it seems unpleasant to you. It's time for a full clean out.
Trust your instinct.

You may want to tweak the substrate if you need to do more than spot clean regularly.

I don't know what you're using. I find that the right ratio of coir & finished compost works remarkably well for my crew. More coir for the tortoises, less for the box turtles. Right is defined by function here. There are other combinations that also work. That's what I mean by tweaking. I don't remember how long it's been since I dumped all the substrate & replaced it, not since I cat proofed it, because it hasn't needed it.

Continue spot cleaning.

Some of us have pill bugs to aid in the cleanup of organic matter.

I change mine once a year, and not all of it. I put pill bugs in it when the flies get heavy. I spot clean daily.

I just go in each morning and pick out any droppings that were deposited the day/night before. I also clean up pieces of old food that didn't get eaten but that is about it.

There really is no set rule of thumb. If it offends you or if it looks dirty to you, clean it out.
