Permanent or fixable?

A bit of both. Whats there already is not fixable but you can make sure any new growth is smooth to stop any further pyramiding. This is done by making sure your husbandry is correct. Food, heat,UV, humidity, etc...

Ditto. What pyramiding has already occurred will always be there, but with the right conditions (wet warm and swampy) you can start to smooth out future growth. How long it takes and how difficult it is will depend on how far you pyramiding has progressed, how much effort you put into stopping it, and your enclosure.

Yes, diet, sun, exercise, hydration and proper environment are all key.
