Odd spot?

Hi. My tortoise Shelly has a really weird spot on her head. It's grey. It feels soft and squishy like the skin on her neck area. It kinda looks like her red scale came off. Any idea what it is? Is she ok?

It looks like some sort of an injury. Like she stuck her head through something to take a peek.
I'd put a bad of general anibiotic on it and then look every day for a few days and see that it improves

Doesn't look too bad to me. I'd watch it for change, but I don't think it warrants a call in to the vet,
