How far should the uvb be?

The best way to find that out is to use a solarmeter, but they are very expensive and not something everyone invests in.
I'll tag @Tom who will be able to help you but meanwhile is there any guidance in/on the box?

Yes, a 4x2 tort box with heating and basking on one side I’ll put the uv in the middle
Repti bark, CHE, 65 watt incandescent floods, plants on cooler side, flat rocks, hide hut, ceramic food and water dish. The box has a cover too this is it open, Tom recommend a cover. So far is there anything else I could need?

Every bulb, fixture and enclosure is different. The only way to set the height correctly is with a Solarmeter 6.5. I run several of these bulbs, and the usual distance is around 18-20 inches. That should put you in the ballpark and not do damage from being too close. You only need to run this bulb for a few hours mid day. No need to run it 12 hours a day.
