I'm not Zovick but it just looks like shedding skin maybe to me...I see nice skin underneath the piece...

Well....your tort is shedding so yeah torts shed...but don't ever pull one off. It should fall off normally. So now @zovick now will come on and politely tell you how wrong I am...lol

I agree with Maggie3fan. That is just dead skin coming off that particular scale on the head. If you wet the area with some warm water you might be able to simply rub on it and get it to come off if the tortoise isn't head shy. You could also try that during or right after a soak which should soften the dead skin a bit.
