    Damn, there is one thing I have forgotten to mention (but my first reply hasn't appeared yet): feed the tort some food high in fibre, bananas work wonders for this. This will smooth up the digestion of the tortoise, most probably fixing the problem.

    Normally you should be very careful with calcium/phosphorus supplements to not go overboard with how much you're giving to the tort. The calcium:phosphorus ratio for torts is 2:1 and should be fed regularly food that respects this ratio. It is good to diversify and giver the tort irregularly...

    Such shapes in the shell are not an issue, they occur naturally and don't look either like pyramiding. Nothing to worry about.

    You could try to feed the tort high moisture food for a while, see if that changes anything. Also try to feed it a bit of garlic to rule out the possibility of worms existing. Keep in mind that food normally stays in the digestive tract of the tort about 48 to 72 hours.

    Take it to the vet but also feed the tort natural dewormers. Garlic works best because that is by far the strongest natural dewormer, even used in humans.

    Both need to eat food with extra calcium, especially the one on the right that seems like the it has the shell becoming transparent. What would the vet tell you is also essential.

    Hatchling outside?

    It could stay under supervision, just leaving it outside of a proper enclosure (especially one that prevents the tort to dig underground and escape) is very risky, tortoises move much faster than one could think they do, especially when they're small.
