what kind of tortoise is this??

i was given this tortoise from a friend and she had no information about him, i was wondering if anyone could help me out by identifying its species. it would help a lot with determining its proper diet and care.
note: i am aware of his shell condition, i'm treating him for it(there's antibiotic cream on his shell, that's why it's gooey hah) otherwise if anyone has any advice on his shell that would be great too

That's a gulf coast box turtle (Terrapene carolina major) that's been kept in the house without the proper diet and UVB. Here's a link to a good care sheet:
