Ornate Box Turtle Head Injury

Hello fellow reptile owners, I've recently found out that my Ornate Box Turtle has a pretty deep scratch on her forehead and this has gotten me really worried. She's about 4-5 months old, I keep her in a 10 gallon Aquarium Tank (Which I'm thinking of improving in size), a substrate consisting of a mix of cypress mulch, sphagnum moss and "Zoo Med Reptibark Natural Reptile Bedding" (which she mostly burrows and sleeps in). I feed her mainly protein based foods along with some veggies (all sprinkled with calcium) and I mist periodically to maintain the humidity. What can I do about her injury and do you think it has to do with her diet/habitat? The things that I thought that could've done this to her were the crickets that I feed her (very unlikely in my opinion), falling off her half cut wood log which she sometimes likes to climb or the reptibark which she loves to dig in head first. There are no other pointy objects that might've scratched her (I think). Please help!

Also this is my first time posting in this forum so if I'm missing anything please tell.

Hi, and welcome to the Forum!

I don't see anything worth worrying about. The 'pit' next to the right eye looks dry like it's healing quite well on its own.

If you're worried, you could put a little bit of triple antibiotic ointment on it once a day after you soak her for 30 minutes.
Can we see pictures of your entire set up, please? That way, if there is something in there that could hurt her, numerous fresh pairs of eyes could help?
