Is this spot on head ok?

I don't know what this is or how to treat it, and he has a runny nose too.

I don't know about that one spot, but I do know he shouldn't have a runny nose.
What are is Temps and humidity?

Humidity is 65 it used to be way lower. The temp is around 80 F.
If that is your ambient humidity, its ok.
But if that is your ambient temperature, it's too low.
This tortoise needs 80 at a minimum. But a basking area of at least another 10 degrees.
Can you photograph your enclosure and show the lights and heating devices so that we can better help you?

That spot just looks like the scale has come off. It's nothing to worry about.

I hired the temp to 90 degrees, but humidity is still the same.
